Methotrexate was developed in the Forties as a treatment for leukemia. While the efficacy of the drug to battle the childhood cancer has been established as minimal, scientists discovered that methotrexate is excellent for the treatment of psoriasis. Obviously, a chemotherapy drug is not the first weapon used against psoriasis. Methotrexate is used on patients who have severe psoriasis that does not respond to other treatments.

The drug can stem the advance of psoriasis and especially the effects of the disease on the liver and kidneys. Treatment with methotrexate is aimed at slowing the excessive cell division, resulting in remissions lasting up to a year or more. Once the treatment is shown to be effective, it is discontinued until the psoriasis returns.

The first lesions of plaque psoriasis appear as red dots that can be very small. These eruptions slowly get larger, producing a silvery white surface scale that is easily shed. When forcibly removed, the scales may leave tiny bleeding points. The most common places to find the scaly patches are on the scalp, elbows, knees and trunk, although they can be found anywhere on the body. The patient needs to understand that new lesions may be induced in normal skin by physical trauma, including scratching the unaffected skin or psoriatic lesions. Due to the generally warm and moist conditions in the vaginal area, psoriatic plaques in the body folds are usually not scaly, but are bright red and fissured. In the United States, it is estimated that three to five million people have psoriasis. There are numerous triggers that are major factors in eliciting lesions. Physical trauma such as rubbing and scratching stimulate the proliferative processes in vaginal psoriasis. An acute streptococcal infection can lead to guttate psoriasis. Stress is also a major factor.

Vaginal psoriasis is an immunologic phenomenon and, as such, is often treated with immunosuppressive drugs like cyclosporine – which is particularly effective in causing a total remission of genital psoriasis. There are many T cells present in psoriatic lesions surrounding the upper dermal blood vessels and therapies are designed in part on suppression of T cells.

Tazarotene is a relatively new topical retinoid developed as another alternative to glucocorticoids, or it can be combined with steroids. There is some indication that tazarotene may prevent skin atrophy.

Psoriasis, at its core, is a biochemical reaction. While normal skin cells take about a month to mature, patients with psoriasis have skin cells that over-multiply, forcing the cells to move up to the top of the skin in less than a week. As the number of cells builds up, the epidermis thickens and the cells pile up in raised, red and scaly lesions. The sometimes-extensive inflammation is caused from the buildup of blood needed to feed the rapidly dividing cells. Alcohol abuse makes psoriasis more aggressive and more difficult to treat and control.

Vaginal psoriasis carries with it a large emotional element. The embarrassment of having psoriasis on the vagina can cause a woman untold misery. Many women avoid intimacy altogether once they have been diagnosed or the first lesion appears. A lack of closeness to another can often cause stress and anxiety, further exacerbating the problem. Teenage girls can be the butt of jokes, and the humiliation of the disease causes female patients to suffer depression at a much higher rate than males.

Chronic, recurrent seborrheic dermatitis is challenging for clinicians due to the lack of safe and effective treatments. Doctors who find that a patient is otherwise healthy but suffers from persistent seborrheic dermatitis believe the disease has a hereditary component that is difficult to battle and impossible to defeat. The best that can be hoped for is effective treatment attempting to manage and limit the symptoms. Seborrheic dermatitis is actually a pre-psoriatic condition – with many patients developing full-blown psoriasis or a mixture of lesions. There is also evidence to suggest that people with severe seborrheic dermatitis have a dramatically increased chance of Parkinson’s disease.

Seborrheic dermatitis in its initial stages is most commonly found on the face, scalp and neck, and is characterized by large areas of yellowish or red and grayish skin with scaly white patches of varying size. On the scalp, it is common dandruff, while the disease causes sticky crusts and fissures in folds behind the ears, and reddish lesions on the trunk. Seborrheic dermatitis is often found in hairy areas – especially spots that include skin folds, and it can affect the genitalia as easily as the rest of the body.

Sunlight seems to play an important role in management of the condition, although a good deal of patients find that anything beyond a few minutes of direct sun exacerbates the symptoms. If symptoms can be relieved or minimized, recurrence most often occurs in the fall – thought to be due to the colder, drier conditions and reduced sunlight. Recurrences can also be linked with alopecia in severe cases. A yeast known as pityrosporon ovale is said to play a role in the pathogenesis of seborrheic dermatitis, and there are indications that topicals containing ketoconazole and selenium sulfide will not cause an adequate response to the yeast. Nutritional deficiencies and a suppressed immune system due to HIV or transplant operations can cause symptoms.

Seborrheic dermatitis is a chronic disorder that requires initial therapy followed by chronic maintenance therapy. Topical glucocorticoid preparations are often effective but can cause atrophy and erythema – especially on the face. Over the counter shampoos containing selenium sulfide or zinc pyrithione are helpful for the scalp dermatitis, and the lather may be used for the face and chest. Tar shampoos are equally effective for many patients. Many times doctors will immediately prescribe a very potent glucocorticoid cream when the disease is discovered, and in severe cases a retinoic acid treatment is used on the face and other exposed regions. Patients using any type of creams or ointments must always be aware of any atrophy that may occur at the treatment site. Researchers have recently made big advances in topical agents that include an anti-inflammatory ingredient said to dramatically shorten outbreaks and more rapidly alleviate the itching and irritation.

When I was a kid, I would get painful, swollen spots on my arms and neck. The plaque psoriasis on my arms was easy enough to cover with long-sleeved shirts, but the flaky, irritated blotches on my neck were absolutely embarrassing. At the time, my parents really did not have the money to get me an extensive work-up at the doctor, so my mom just rubbed some hydrocortisone on the eczema psoriasis to try and control the incessant itch. Today, the psoriasis has caused further complications in my body, and the outward physical manifestations of the disease are the least of my problems. People with plaque psoriasis or psoriasis dermatitis can often develop psoriatic arthritis, and about five years ago I sure did. At first it was just the ends of my fingers that would get stiff and sore. Soon my wrists ached a lot and my hands felt weak and sore overall. Over the course of several months, the pain crept into my spine. Soon my back pain was chronic and I was miserable. My doctor gave me some ibuprofen that was no help so he got me on some Vicodin, and no drinking since alcohol and psoriasis do not mix at all. The narcotics really took the edge off, but I could not be doped up all the time. I had to drive and work so the pills had to be limited. Now with Humira psoriasis is not eating away at my joints anymore. My daily activities have become reasonably routine again, without having to move slowly or stop sooner than I would like to. And best of all, I do not have to take dangerous pain killers.
